16.2.1. ED Discrepancy Workflow
The workflow for ED discrepancy is as follows:
The ED physician makes a preliminary report on the radiology exam by setting it as an “emergency” and documenting any normal or abnormal findings.
The responsible radiologist is notified when they log onto the PACS and can perform an “ED overread” to verify the findings on the preliminary report and make any changes if needed.
3. The radiologist will look for any discrepancies on the report and decide when a follow-up would be required for the patient (if any) based on these categories:
[1] Agree: Discrepancy implies a low risk to the patient with follow-up not required for several days.
[2] Minor: Discrepancy implies moderate risk with follow-up needed in the next day or two.
[3] Major: Discrepancy is one that is potentially serious requiring immediate follow-up.
4. The ED physician is then alerted of any changes made by the radiologist and appropriate follow-up action is tracked until completed.
